Twenty-eight Real World alumni are brought to Maturín Venezuela for a whole new game. Each player will play in multiple challenges followed by elimination rounds held in The Bastion. After every challenge, the winners gain immunity from elimination and nominate three players of each gender for the Guillotine. The rest of the house then votes on one of the last-place players from each gender to go straight into the Bastion.

34 competitors are divided into two teams based on sex. The two teams compete against each other in a challenge. The worst person on each team, as determined by the challenge, automatically goes into elimination, called "The Cube". The top three on each team form the "Inner Circle". The Inner Circle then decides who, from the opposite team, goes into elimination against the day's loser. Members of the Inner Circle from both teams are immune from being voted in. One for the team captains and decide who will be safe from elimination and the second is the team challenge.Which ever team wins the challnge also banks $5,000 into their team account.
